(Canine Only) Has your dog had the DAPP vaccine? (Distemper, Adenovirus, Parainfluenza, and Parvo)
Where was the vaccine administered?
Has your pet received a rabies vaccine?
(Feline Only) Has your cat had the FVRCP vaccine?
CPR: If your pet requires cardiopulmonary resuscitation (CPR), do you wish this procedure to be performed? Cost can be up to $500.
Do you grant permission for blood work and radiology to be done before the doctor contacts you with a treatment plan?
Estimated cost $420 non same day treatment/non emergencies. $460 for emergencies/walk in. Please note that this is only exam and basic diagnostics.
